CDS4 and CDS5 eForms mandatory from 1 April 2013

Paper versions of two of our crime forms will be rejected after 31 March 2013

Providers who use CDS4 and CDS5 crime forms need to make sure they are now ready to make electronic submissions.

CDS4s - Application for prior authority to incur disbursements in criminal cases

CDS5s - Application for extension of upper limit

(These are to be renamed CRM4 and CRM5 respectively from 1 April 2013)

From 1 April 2013, if you submit paper versions of the above forms, they will be rejected and you will be asked to resubmit them online.

Paper submissions will only be accepted if signed on or before 31 March and received on 30 April.

Registration is simple

We ask you to register now to use the eForms so:

  • you can familiarise yourself with how the eForm system works 
  • we can work together to ensure you have no issues 

To make registration quicker we suggest that your CWA administrator carries out the same administration role for eForms ? our eForms use the same software system as CWA.

Form name change

From 1 April 2013, all our crime forms will start with the prefix CRM so CDS4 and CDS5 will be renamed CRM4 and CRM5 respectively. There will be no paper CRM4 or CRM5.
