Prison officials who failed to save an Asian teenager murdered by his white racist cellmate should be named, an inquiry has been told.
In closing statements to the inquiry into Zahid Mubarek's murder, counsel Nigel Giffin QC said staff had up to 15 chances to change the course of events.
Robert Stewart killed Mr Mubarek at Feltham Young Offenders' Institution, west London, in March 2000.
