CDS4 and CDS5 eForms become mandatory from 1 April 2013.
Registration for CDS4 and CDS5 eForms is simple and there are benefits for you in registering now.
- You can familiarize yourself with the eForm system before it becomes mandatory
- We can work with you to resolve any issues in advance.
We recommend that your CWA administrator does your eForm administration because our eForms use the same software system as CWA.
Manual CDS4 & CDS5 forms rejected from 1 April
Any CDS4s and CDS5s signed on or after 1 April 2013 will be rejected and you will be asked to resubmit the application online as an eForm.
CDS4s and CDS5s signed on or before 31 March will be accepted up to 5pm on 30 April.
All relevant documents must be scanned alongside your claims.

Form name change?
From 1 April, all our crime forms will start with the prefix CRM so CDS4 and CDS5 will be renamed CRM4 and CRM5 respectively.
There will be no paper CRM4 or CRM5.
