Register now so you are ready to submit crime forms online

You will only be able to submit CDS4 and CDS5 forms electronically from 1 April 2013

Register now so you are ready for mandatory online working from 1 April 2013 with two of our crime forms:

1. CDS4s - application for prior authority to incur disbursements in criminal cases
2. CDS5s - application for extension of upper limit (CDS5)

The LSC is committed to an agenda to increase online services - eforms is part of that.

Using eforms speeds up our processing times and reduces cost.

1 April 2013 - mandatory deadline

All CDS4 and CDS5 forms must be submitted electronically from 1 April 2013.

We would encourage you to register to use the eforms now so:

  • you can familiarise yourself with how the eform system works
  • we can work together to ensure you have no issues 

To make registration quicker we suggest that your CWA administrator carries out the same administration role for eforms (our eforms use the same software system as CWA).

Manual rejections from 1 April 2013

From 1 April 2013, if you submit a paper version of the above forms, they will be rejected and you will be asked to resubmit them online.

Submitting other documents

It is important that all relevant documents are scanned alongside your claims.
