CRM4 & CRM5 eForms become mandatory

Paper versions of the renamed CRM4 and CRM5 crime forms will be rejected from 1 April 2013 and you will be asked to resubmit them online.

Paper submissions will only be accepted if signed on or before 31 March and received by 30 April 2013.

From 1 April, providers need to make electronic submissions using these eForms.

CRM4 = application for prior authority to incur disbursements in criminal cases
CRM5 = application for extension of upper limit

All relevant documents must be scanned alongside claims.
