Frances Andrade's suicide 'could and should' have been prevented
PUBLISHED April 10, 2014The death of Francis Andrade, the violinist who committed suicide days after giving evidence against her abuser, "could and should" have been prevented and the agencies who failed her must improve their treatment of vulnerable witnesses, a serious case review has found.
